I have 2 friends who had prostate cancer and had surgery to remove the cancer. One friend almost died because of a mistake done in surgery and it took him 2 years to fully recover . My other friend now suffers from incontinence and has to wear an adult diaper all the time because of a mistake done in surgery.  They both now say they should have researched laser treatments.

Harvard Medical School releases 2010 Annual Report on Prostate Diseases

Harvard Medical School today released its first ever Annual Report on Prostate Diseases, which covers the basics in the diagnosis and treatment of prostate cancer, benign prostatic hyperplasia (BPH), erectile dysfunction, prostatitis, and related issues, along with recent scientific and medical advances. A group of talented prostate disease specialists contributed to the publication.

Also included in this groundbreaking report is an explanation of the growing body of evidence indicating that the benefits of PSA screening may not outweigh the potential harm of unnecessary treatment, a concept that counters current medical practice.

Given the ongoing debate, Dr. Marc Garnick, editor in chief of the 2010 Annual Report on Prostate Diseases, invited three experts to participate in a roundtable discussion on screening. The trio shared their thoughts on PSA screening and explained its pros and cons in easy-to-understand terms. A PSA screening has always been somewhat controversial. There is good news for sufferers of BHP (Benign Hypertrophy of the Prostate) and other prostate conditions – new laser treatments are available that have significantly less side effects than the previous treatments (TURP/TUNA), and much shorter recovery durations. In addition, the new laser techniques are ideal for patient who are on blood thinning medications – a not uncommon coincidence, as both conditions tend to crop up in men over fifty.

There are two competing laser technologies for prostate treatments – you should understand the differences: the Holmium (YAG) laser, and PVP (KPT) laser (otherwise known as “Green Light”.) The differences between the two are in the wavelength of the lasers used, and the nature of the energy transmitted to the tissues.

PVP LASER (Green Light)

The PVP (KPT) laser has a somewhat long wavelength that penetrates the tissue more deeply – in fact, the energy penetrates more deeply than it ablates (removes). The consequence is that unablated remaining tissue has been affected by the laser. The result can be a phenomenon known as necrosis of the affected tissue: the remaining tissue dies and may require further treatment of other means to remove, down the line.

This can be a serious side effect. Because of the higher possibility of necrosis, PVP is not recommended for larger prostate conditions. It is considered viable for smaller prostates.

HOLMIUM LASER

The Holmium (YAG) laser has a shorter wavelength and penetrates less deeply than the PVP. Energy from the laser is imparted only to the the tissue that is ablated (removed). As a result, there is no necrosis concern.

In a discussion comparing the two technologies (for HoLAP application)[1], a panel of noted urlogists universally concluded the Holmium laser to be preferable to. the PVP laser.

HoLEP – ENUCLEATION OF THE PROSTATE

With the Holmium laser, it is now possible to completely remove the prostate with minimal side effects. This is called enucleation. The prostate is completely detached with fine (2-3mm) laser instruments, and passed through a pair of incisions in the bladder. It is then sectioned, and the sections are brought into the bladder through the incisions, where they are broken up into passable bits (the bits are passed through the catheter.)

This is becoming the new “gold standared” treatment, replacing the TURP procedure in many advanced urological centers. Side effects are minimal, and, according to one study[2], sexual function (ability to gain and maintain an erection) may actually increase, depending on the nature of prior dysfunction, if any.

WHERE TO GO?

One consideration is going overseas for treatment. HoLEP is available in at least one group of urolotical hospitals located in India, using exactly the same procedures and equipment as in major US facilities. Costs overseas can be significantly less than that in the US.

As anyone who has been to see a doctor knows, the medical community is full of jargon and abbreviations that generally tend to confuse (and often annoy!) the non-medical people… who are the ones being treated! One of these abbreviations, “BPH” is quite frequently heard when the patients are males over the age of 40. BPH stands for “Benign Prostatic Hyperplasia,” or an enlargement of the prostate gland. This finding is observed in roughly a quarter of all men who fall into this age group, so it is not a rare occurrence. When put into layman’s terms, BPH means three specific things:

“Benign” means that the enlargement of the prostate gland is NOT cancerous! From the outset, that is the very good news. It must also be noted that BPH is an abnormal enlargement, however, and it should be treated. Taking care of the problem early is important in order to alleviate symptoms and prevent the development of more serious problems later on.

“Prostatic” simply refers to the prostate gland itself. BPH means that the prostate gland has increased in size and may be pressing on the urethra, which is the “tube” that leads from the bladder and allows urine to escape the body. Naturally, when the urethra is constricted, it can mean difficulties for someone when it comes to urinating – whether it affects frequency or the ability to completely empty the bladder. Because of the increased pressure on the urethra, BPH can also adversely affect the sexual health and performance of a man.

“Hyperplasia” means that the prostate gland is enlarged due to organized, but abnormal, growth of the cells that make up the gland. It is this enlargement that leads to the symptoms listed above. Fortunately, there are a number of treatment options for men who have BPH. Some of these include over-the-counter supplements and remedies, which should be discussed with a doctor along with pharmaceutical drugs or surgeries that may help the situation. Men with benign prostatic hyperplasia and they have difficulty in passing urine. Enlargement of prostate gland increases in course of a lifetime putting pressure on the prostatic urethra. It may become more annoying to men and be difficult to control urine. Men may start leaking even when the urinary bladder contains small amount of urine.

Few of the symptoms of benign prostatic hyperplasia includes

(a)Difficulty while starting to urinate(b) Straining while trying to urinate(c) Dribbling of urine(d) Frequent urination(e) Flow of urine with low pressure(f) Starting to urinate minutes after passing urine(e) Urinating during night frequently(f) A feel of incomplete bladder emptying

Drugs for prostate enlargement are prescribed during the initial stage of the disease, but during the latter stage of disease, urinary bladder muscles become stronger and thicker. This leads to urine remaining completely in the bladder. It may not respond to drugs and the symptoms do not alleviate, at this stage surgery or other treatment option is required to have a complete cure.

Green light laser is used to treat patient suffering with enlargement of prostate gland or Benign Prostatic Hyperplasia. It is an invasive procedure that evaporates the prostate tissue completely, but there are many complications that arise during this procedure. High power laser energy is used to dissolve the prostate tissue, the rays are absorbed by the prostate tissue and it is followed by the complete vaporization and removal of the tissue. Green light laser procedure:

the indications and contraindications of laser TURP is as belows:Surgical Indications(1) The obstruction is caused by benign prostatic hyperplasia symptoms and signs, such as nocturia more times, difficulty urinating, urine flow thinning, interrupted urination. An increase in residual urine of more than 60ml or urinary retention occurred(2) determined to cerebral vascular complications in patients with stable angina takes at least three months, after examination myocardial infarction patients recovered would require six months in patients with cerebral vascular accident in stable condition after 2 months, diabetic patients in the cancer is under control before underwent transurethral resection of the prostate. Combined urinary retention, urinary tract infection in patients with drainage of urine which should firstly control the infection, before the transurethral resection of the prostate.(3) benign prostatic hyperplasia glands in patients are with less than 60g suitable for transurethral resection of the prostate laser TURP.Contraindication for surgery patients(1) cardio-cerebral vascular disease: severe hypertension, acute myocardial infarction, failed to control heart failure, recent hemiplegia is due to cerebrovascular accidents.(2) respiratory diseases: severe bronchial asthma, emphysema, pulmonary infection, lung function significantly impaired.(3) a serious liver and kidney dysfunction.(4) systemic haemorrhagic disease.(5) severely diabetic condition has not been controller.(6) acute urinary tract infection in the reproductive system were not brought under control.(7) The proliferation of the gland is too large to do more than 60g,which are generally not suitable for transurethral resection of the prostate due to a longer operative time, bleeding is relatively large. But for those who experienced surgery,which is not an absolute contraindication.(8) patients with pacemakers should not accept this procedure in general. However, some people do not think it’s as contraindication.Transurethral resection of the majority of contraindication is relative, patient’s condition is improved after treatment, in closely monitored conditions that can be done transurethral resection of the prostate laser TURP surgery.
